Who we are
BoneForte helps you find REMS certified medical providers and book bone health scanning appointments. This policy explains what information we collect and how we use it.
Information we collect
- Search activity: location, provider preferences, and other search inputs you enter.
- Appointment requests: your name, date of birth, phone number, email address, and any other details you provide when booking.
- Technical data: IP address, browser type, and basic usage data collected automatically.
How we use your information
- To show you providers in the BoneForte network and let you book an appointment.
- To send you appointment confirmations and reminders.
- To operate, secure, and improve the site.
Sharing your information
When you book an appointment, BoneForte shares your contact and appointment information with the provider you selected so they can prepare for and deliver your care. That provider is a HIPAA-covered entity and will handle your information under its own Notice of Privacy Practices.
We do not sell your personal information. We do not share it with advertisers. We share it only with the provider in the BoneForte provider network, with service vendors that help us run the site under confidentiality agreements, or when required by law.
How we protect your information
Your data is transmitted and stored using encryption and access controls consistent with HIPAA requirements. We keep appointment information only as long as needed to complete your booking and meet legal obligations.
Your choices
You can contact us to correct or delete the information we hold about you, or to opt out of reminder messages. Requests about your medical records should go directly to your provider.
Children
BoneForte is not intended for use by anyone under 18 without a parent or guardian.
Changes
We may update this policy from time to time. The date at the top shows the current version.
